Any-

   (a)  notice given under section 20B(1) of the Ordinance; or

   (b)  information given under section 20B(3)(b) of the Ordinance, shall
        contain the information specified in Schedule 2, with such variations
        as the circumstances of the case may require. (L.N. 34 of 2003)
