Application for inclusion in the list

PART VII

INCLUSION IN THE LIST AND ALTERNATIVE QUALIFYING REQUIREMENTS

(1) An application under section 90 of the Ordinance shall-

   (a)  be made in such form as decided by the Board;

   (b)  be delivered to the Board secretary, together with 2 copies of a
        recent photograph of the applicant; and

   (c)  be accompanied by evidence showing that the applicant was practising
        Chinese medicine on 3 January 2000.

(2) The applicant must produce his Hong Kong Identity Card or another document
of identification on request for verification by the Board secretary or a
person authorized by him.
