Additional information for imported organs

For the purposes of section 7(1) of the Ordinance, the following additional
information shall be supplied in a certificate for an imported organ-

   (a)  type of organ and the quantity;

   (b)  name, age and sex of donor;

   (c)  date of removal of organ;

   (d)  where the donor is deceased-

        (i)    time and date of death;

        (ii)   cause of death (if known);

   (e)  name of airline or carrier by which it is to be sent;

   (f)  date on which it is to be sent.
