OPTOMETRISTS (REGISTRATION AND DISCIPLINARY PROCEDURE) REGULATION - SECT 7

Application for registration or provisional registration

(1) An application for registration as an optometrist under section 13 of the
Ordinance or for provisional registration as an optometrist under section 15
of the Ordinance shall be in accordance with Form 1 in Schedule 2.

(2) A form of application for registration or provisional registration shall
be signed in the presence of an optometrist registered in Part I of the
register, barrister, solicitor, registered medical practitioner,
registered dentist or commissioner for oaths and shall be delivered to the
Secretary together with 4 photographs of the applicant of such size as shall
be specified by the Secretary. (47 of 1997 s. 10)

(3) The Secretary shall refer the application of a person claiming to be
qualified to be registered-

   (a)  under section 12(1)(a) or 15 of the Ordinance, to the Board; and

   (b)  under section 12(1)(b) or (c) of the Ordinance, to the Council, as
        soon as practicable after receiving the application.

(4) In subsection (2) "registered dentist" (註冊牙醫) has the meaning
assigned to it by section 2(1) of the Dentists Registration Ordinance ( Cap
