3. Business receipts, by type.

4. Number of persons engaged.

6. Descriptions and prices of services rendered to customers. (L.N. 290 of
1998)

Where information in respect of the above matters is given in relation to
business establishments by furnishing a combined questionnaire, the
information in respect of items 1, 2 and 4 is required to be given in relation
