Form of certificate application and orders

PART V

MISCELLANEOUS

For the purposes of section 29 of the Ordinance-

   (a)  a certificate shall be in Form 10;

   (b)  a prohibition order shall be in Form 11;

   (c)  an application for the extension or renewal of a prohibition order
        shall be in Form 12;

   (d)  an order under subsection (4) thereof shall be in Form 13;

   (e)  an application under subsection (5) thereof shall be in Form 14;

   (f)  a notice under subsection (6) thereof shall be in Form 15; and

   (g)  an order under subsection (7) thereof shall be in Form 16, in the
        Second Schedule. (Enacted 1985)
