BIRTH CERTIFICATE (SHORTENED FORM) REGULATIONS - REGULATION 4
Particulars to be furnished by applicants
An applicant for a short birth certificate to which these regulations apply
shall furnish to the registrar at the general register office, except where
the application is made to him at the time of registration of the birth, the
following particulars relating to the registered person-
(a) his surname and name; (L.N. 498 of 1995)
(b) the date and place of his birth;
(c) the surname and name of his father; and (L.N. 498 of 1995)
(d) the maiden surname and name of his mother: (L.N. 498 of 1995) Provided
that the registrar at the general register office may dispense with
the furnishing of any of the said particulars which in his opinion it
is not reasonably practicable for the applicant to furnish.
