DENTISTS (REGISTRATION AND DISCIPLINARY PROCEDURE) REGULATIONS - REGULATION 5

Application for registration

(1) An application for registration shall be-

   (a)  if the applicant is resident in Hong Kong-

        (i)    in accordance with Form 2 in the First Schedule; and

        (ii)   completed in the presence of a barrister-at-law, commissioner
               for oaths, minister of religion, registered dentist or
               solicitor; or (47 of 1997 s. 10)

   (b)  if the applicant is resident outside Hong Kong-

        (i)    in accordance with Form 2A in the First Schedule; and

        (ii)   completed in the presence of a commissioner for oaths or notary
               public.

(2) An application under paragraph (1) shall be delivered to the Registrar,
together with 4 copies of a photograph of the applicant of a size not greater
than 50 X 70 mm and not less than 40 X 60 mm. (L.N. 2 of 1985) (L.N. 177 of
