Identification of licensees

Every person applying for a licence shall furnish to the Commissioner 2 copies
of the photograph of-

   (a)  in the case of an application by an individual person, himself; and

   (b)  any other person for the time being in charge of the licensed
        premises, of a reasonable likeness to the satisfaction of the
        Commissioner and of a size not greater than 50 mm by 50 mm and not
        less than 40 mm by 40 mm, and shall personally sign an application for
