CHINESE MEDICINE ORDINANCE - SECT 84

Application for limited registration

(1) An application for limited registration shall be made to the
Practitioners Board in such form as the Practitioners Board may determine and
shall be accompanied by-

   (a)  such documents and particulars as may be determined by the
        Practitioners Board; and

   (b)  a prescribed application fee.

(2) For the purpose of determining an application for limited  registration,
the Practitioners Board may request the person on whose behalf the application
is made to submit a declaration stating the same particulars as are required
under section 68(3)(a).
