To the Registrar of Marriages， Hong Kong, 致香港婚姻登記官
I hereby give you notice that a marriage is intended to be had， within 3 months from the date hereof， between me and the other party herein named.
本人與名列下表的 另一方， 擬在 今天起計的 3 個月內結婚， 特此通知。

鑑於A.B. 及C.D. 意欲結婚， 並有充分理由使本人相信該婚姻應予許可而無須先經《 婚姻條例》 (第181章)訂明的 程序，
因此本人現依據該條例， 免除其
中訂明的發出通知及發給證明書程序〔或視情況而定免除其中一項〕，並授權合資格的人在 19 年 月
日上午 時至下午 時之間在〔舉行婚禮地點〕為上述的A.B. 及C.D. 主持婚禮。

， 現聲明︰ 本人完全明白， 經過在 婚姻登記官面前當眾表示以C.D.為妻子〔 或 丈夫， 視情況而定〕 後，
雖然沒有世俗或 宗教儀式， 本人與上述的 C.D.即結為合法夫妻。 本人亦明白， 經以上過程當眾表示以上述的
C.D.為妻子〔 或 丈夫， 視情況而定〕 後， 我倆因此而締結的 婚姻， 在 我倆共同在 生之時， 非經法 庭有效判定離婚，
不得解除； 而這段婚姻一日未解除之際， 任何一方於對方在 生時締結另一段婚姻， 即犯重婚罪， 可處以該罪行的
刑罰。
